RE: Vaccine Appointment Assistance

Dear Resident: 

I am pleased to announce a partnership with Centrastate Medical Center in Freehold to provide the COVID-19 vaccine. In order to be eligible you must be:

  • A Marlboro resident
  • Meet state criteria, including age 55 and over, additional essential workers, and individuals with intellectual and developmental disabilities. 

If you would like to be scheduled for the vaccine, please complete the questionnaire linked here: https://cities.formstack.com/forms/marlboro_covid_19_registration no later than Friday, April 16, 2021. 

You will be contacted directly by Centrastate to schedule your appointment.  The vaccine will be either Pfizer or Moderna and will be given at Freehold Centrastate Medical Center.  Please bring your insurance card and identification. 

If you require transportation to your vaccine appointment, wait until your appointment is confirmed and call SCAT at Monmouth County Division of Transportation at 732-431-6480 and choose option 1 to schedule your ride.  

We are starting to see the light at the end of the tunnel, and it is our hope that helping residents access vaccines will get us there sooner rather later.
